Aceclofenac is used to relieve pain and inflammation of joints and vertebrae in conditions such as osteoarthritis, rheumatoid arthritis and ankylosing spondylitis.
The most common side effects are gastrointestinal in nature including ulcers in the mouth, stomach and small intestine, urge to vomit, vomiting, diarrhea, constipation, excessive passage of wing or gas, indigestion, abdominal pain, blood in stools. Other common side effects are swelling of part or whole of the body, increased blood pressure, heart failure, and severe allergic reactions.
Aceclofenac may increase concentrations of lithium, digoxin and methotrexate in the blood, increase the activity of anticoagulants (warfarin), reduce the activity of diuretics (furosemide) and anti-hypertensives, cause increased toxicity in kidney (cyclosporine). Interaction with quinolone antibiotics (ciprofloxacin, levofloxacin), anti-diabetic drugs, pain killers (aspirin, ibuprofen), or corticosteroids can cause adverse effects. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
MECHANISM OF ACTION
Aceclofenac works by blocking the effect of chemicals called cyclooxygenase enzymes. These enzymes help to make other chemicals in the body, called prostaglandins. Prostaglandins are produced at sites of injury or damage, and cause pain and swelling. By blocking the effect of cyclooxygenase enzymes, production of prostaglandins are reduced, thus, pain and swelling are eased.
Paracetamol is used for the relief of mild to moderate pain in conditions such as headache (including migraine), toothache, ear pain, joint pain, pain of menstruation, fever, feverish colds, and flu.
Side effects of paracetamol are usually rare, and include allergic reactions such as shortness of breath, abnormalities of blood cells, skin reddening, blisters or rash, swollen facial features, or a runny nose. Overdose (more than 4 g in adults and more than 2 g in children) may cause severe liver damage.
Cholestyramine, metoclopramide and domperidone may alter the effects of paracetamol. Use of paracetamol with warfarin may increase the risk of bleeding. Paracetamol may increase the effects of chloramphenicol. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
MECHANISM OF ACTION
Paracetamol is categorized as an analgesic (painkiller) and antipyretic (fever reducing) drug. Paracetamol reduces pain by increasing the pain threshold. It reduces fever by acting on the temperature-regulating region of brain, resulting in increased blood flow across the skin, sweating and heat loss.
Tramadol is a centrally acting analgesic, which possesses opioid agonist properties. Tramadol also appears to modify the transmission of pain impulses by inhibition of monoamine reuptake.
Dizziness, headache, urge to vomit (nausea), vomiting, constipation, unusual tiredness (fatigue), drowsiness, dry mouth and sweating.
Tramadol is known to interact with other drugs like cimetidine, carbamazepine, pentazocine, nalbuphine, buprenorphine, ondansetron, , monoamine oxidase inhibitors (seligiline, phenelzine), bupropion, mirtazapine, tetrahydrocannabinol, antidepressants or antipsychotics, sedative medicines such as tranquilisers, or sleeping pills, and other pain relievers (morphine, codeine, oxymorphone), medicines that inhibit blood clotting (warfarin). Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.

Tramadol reduces the pain through its action on specific nerve cells in the spinal cord and brain.

Experiences of drug use and evidence from studies in pregnant women have shown risk to fetus. This drug may be used in pregnant women if benefit outweighs the risk. Patients should follow the advice of the doctor regarding its use.
