TYPICAL USAGE

Clonazepam is used to treat fits (epilepsy or seizures) and sudden, unexpected attacks of extreme fear, and worry about these attacks (panic disorder) with or without the fear of public places (agoraphobia). 

SIDE EFFECTS

Allergic reactions such as skin rash, flaking skin, boils, sore lips and mouth, swelling of the face, fever, sudden wheezing, fluttering or tightness of the chest or collapse, being aggressive, excited, irritable, nervous, agitated or anxious, problems sleeping, nightmares or vivid dreams, thoughts of harming or killing yourself, mental problems such as seeing or hearing things that are not really there (hallucinations), believing in things that are not real (delusions) or problems with your speech, types of fits (seizures) that you have not had before, drowsiness, tiredness, dizziness, light-headedness, restlessness, poor concentration, confusion or a feeling of being lost (disorientation), difficulty remembering new things, headache, depression, weak or floppy muscles or jerky movements (poor coordination), feeling unsteady when walking, salivary or bronchial hyper secretion with drooling in infants and children (especially those with mental impairment), abnormal results of liver function blood test, urge to vomit (nausea), stomach upset, loss of bladder control, bruising easily, being short of breath and nose bleeds, double vision, jerky movements of the eyes (nystagmus), hair loss, loss of sex drive, difficulty getting or maintaining an erection (erectile dysfunction)If clonazepam is stopped suddenly after prolonged use you may experience the following symptoms (Withdrawal symptoms)Problems sleeping, muscle pain, uncontrolled shaking (tremor) or feeling restless, feeling very anxious, tense, confused, irritable or agitated, or changes in your mood, increased sweating, headache, feeling sensitive to light, noise or physical contact, seeing or hearing things that are not really there (hallucinations), tingling and feeling numb in your arms and legs, feeling of losing contact with realityConsult your doctor, if you need urgent medical treatment for any of the above side effects

DRUG INTERACTION

Clonazepam may interact with  * Herbal medicines  * Other medicines to treat fits (e.g.carbamazepine, hydantoins, phenobarbital, phenytoin, primidone or sodium valproate) * Medicines used to make you sleep (hypnotics; e.g. zolpidem, zopiclone, temazepam etc) * Medicines that help with anxiety (tranquillizers; e.g. alprazolam, chlordiazepoxide) * Pain killers (e.g. diclofenac, aceclofenac, ibuprofen etc) * Medicines to relax muscles (muscle relaxants) * Medicines for stomach problems and heartburn (e.g. cimetidine) * Medicine used to treat tuberculosis (e.g. rifampicin) Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.a

MECHANISM OF ACTION

Clonazepam belongs to a group of medicines called benzodiazepines. The exact mechanism of action of clonazepam is unknown; it is however believed to enhance the action of a signaling chemical called gamma aminobutyric acid (GABA) in the brain, which helps to calm the excessive electrical nerve activity that is responsible for causing fits (seizures) and panic disorders.

DOSAGE

Always follow the instructions of doctor regarding dosage, duration and frequency  * Adults:  0.25mg to 5mg 2-3 times daily  * Infants and children (up to 10 years of age or 30 kg of body weight): 0.01 and 0.03 mg/kg/day in two or three divided doses. Dosage may be increased up to a maximum of 0.05 mg/kg/day.

* Do not drive, operate heavy machinery, or perform activities that require alertness * Report any confusion or inability to control muscle movements to your doctor  * Clonazepam may be habit forming. Keep the medication in a place where others/ children cannot get to it. * Do not take alcohol while you are on this drug  * Do not take clonazepam               -If you are allergic (hypersensitive) to clonazepam or any of the drugs ingredients and                other benzodiazepine medicines (e.g. diazepam, flurazepam, temazepam)              -If you have breathing problems or lung disease               -If your breathing stops when you are asleep (sleep apnea)              -If you have severe liver problems              -If your muscles become weak and get tired easily (myasthenia gravis) * Do talk to your doctor or pharmacist:               -If you have any liver, kidney or lung problems              -If ever had depression or thoughts of harming or killing yourself              -If there is any recent death incidence of friend or family              -If you are pregnant, planning to become pregnant or are breastfeeding              -If there is a current and past use of alcohol or other drugs of abuse              -If you are shaky and unsteady, have slurred speech or have rapid eye movements                    (spinal or cerebellar ataxia)              -If you have a rare, inherited blood problem called porphyria             -If you are taking or have recently taken any medicines without prescription including                    herbal medicines             -If you are planned to receive anesthesia for any operation or dental procedure             -If you have to drive or use machines or tools while taking clonazepam as clonazepam               can slow down your reactions * Precaution must be taken, when receiving treatment with other medications that act on the brain or other drugs used for the treatment of fits (e.g .phenytoin, phenobarbitone etc.)
